To request a copy of your records, please contact VeriTrust. Fees will apply for copying and mailing. To transfer your records to your new doctor, sign a disclosure authorization with VeriTrust. If you need an office visit, prescriptions, assistance with pending referrals, or help with pending lab orders or exams, please contact our office before August 14, 2026, so we can assist you. Your medical records are confidential and only can be transferred to others with your permission.
After August 31, 2026, if you have a medical emergency, call 911 or go to the nearest Emergency Room.
